Four steps, and most of your part is the first one.
1. Answer a short online questionnaire. No video visit, no appointment to book. Tell us what happened and send the form or DLS message you received — you don't need to know its name.
2. A state-licensed provider reviews your answers, then completes and signs the exact form DLS sent you — field by field, with the functional detail it asks for.
3. The completed form comes back to you by email. We can also send it directly to DLS after you complete an authorization.
4. If DLS writes back wanting verification, clarification, or corrections, that follow-up is handled at no additional charge.
Which DLS forms can you complete?
Three, plus the follow-up that comes after them:
- Health Care Provider Form
- Leave as an Accommodation
- Physician Statement / RFI
An RFI is a request for additional information — DLS asking for something it doesn't have yet. It can follow any of these forms, not just the Physician Statement. A state-licensed provider completes the exact form DLS sent you, field by field. Using the DLS form is often the clearest way to provide every item requested and reduce follow-up.
Those are the forms we see most. If DLS sent you a different form, a state-licensed provider can complete that one too.
What if my doctor won't fill out the DLS form?
Common, and not your fault. Urgent care and telehealth apps often say they don't complete employer forms, and a regular doctor's office may charge per form and book weeks out. That refusal is the reason Certicare exists. A state-licensed provider completes the DLS form after a real review of your answers — no appointment to book, no waiting weeks for a slot.
How fast will my paperwork be done?
Usually within 24 hours after we have everything needed: your questionnaire answers and the form DLS sent you. The clock starts when the file is complete, not when you open the questionnaire.
Start whenever. The questionnaire is online now, and the completed form arrives by email. If it's the middle of the night, begin anyway — you don't have to reach anyone to start.
Email and phone replies usually come the same day. Anything after 6 pm ET is usually answered the next morning. Check the deadline in your DLS message, and give us the file with room to work.
What does it cost?
$49 flat, shown before you start — one fee for this request: the DLS form you were sent, plus verification, clarification, or corrections on that form. If the form can't be completed, you get a full refund.

What happens if DLS says my documentation is insufficient?
"Insufficient" usually means DLS doesn't yet have what it asked for: entries missing, answers vague or nonresponsive, or a generic document like a visit summary in place of the item requested. It doesn't necessarily mean the wrong form.
If that happens, the follow-up is handled at no additional charge. A state-licensed provider completes the form to its requirements and answers DLS's follow-up requests. The decision still belongs to Amazon; the paperwork is the part that's fixable.
What we can't do

We can't promise DLS or Amazon will approve your leave or accommodation. Amazon makes that call. What a state-licensed provider can do is complete the form to its requirements, so the request isn't held up over missing paperwork.
We can't produce documentation without a real evaluation. Every form follows an actual review of your answers by a state-licensed provider. There is no version of this where a completed form arrives without the review behind it.
We can't falsify dates or document anything a provider can't clinically support. A past absence can still be evaluated; the form shows the date it was actually completed. If your situation isn't something a provider can support, we'll tell you, and you get a full refund.
