UPT is Amazon's automatic time-off bank, the number you watched drop in A to Z. A negative balance can put you into an attendance review. If the missed time was medical, you open or update a DLS case, and DLS asks for documentation. Usually a specific form, completed by a clinician, by the deadline in your case message. A plain doctor's note usually isn't enough.
Will I get fired for negative UPT?

What if my doctor won't fill out Amazon's form?
It's common. Urgent care visits and telehealth appointments often refuse employer paperwork, and a regular doctor's office may charge extra and only complete forms for established patients. Weeks you don't have.

What does DLS actually need?

If DLS comes back asking for verification, clarification, or a correction, that follow-up is handled at no additional charge. We can also send your form directly to DLS, if you'd rather we handle that step.
Is this a real evaluation, or paperwork from a template?
It's a real evaluation. The provider reviews your symptoms, dates, and limitations, and completes the form to what your situation supports. If it can't be medically supported, they tell you and you're refunded. Nothing is filled out from a template.
Can Certicare guarantee DLS will accept my form?
No. Amazon makes the final decision on leave and accommodation requests. What we can do is have a state-licensed provider complete your form accurately and completely, which is what DLS is asking for.
I already have a doctor's note. Isn't that enough?
Usually not. DLS forms ask for specific details: dates, functional limitations, a provider's signature, things a general note doesn't include. That gap is often why documentation gets marked insufficient.

Yes, when the evaluation supports it. A state-licensed provider can complete paperwork today for a past absence, based on your intake and any records you provide. It's dated for the day it's completed, and it only covers what the evaluation supports.
