UPT is Amazon's unpaid time off balance. A negative UPT balance means your account shows more unpaid time used than was available. Check the dates and deductions in A to Z. If an entry looks wrong, ask HR to review it. For medical absences, ask DLS which leave options and documentation apply.
Will I get fired for negative UPT?
Your UPT balance alone does not tell us what Amazon will decide. Read any attendance notice and respond by its deadline. Ask HR about a balance error or a nonmedical absence. If missed time was medical, contact DLS and follow the instructions for your case. A completed medical form supports a leave request; it does not guarantee approval, restored UPT, or continued employment.

What if my doctor won't fill out Amazon's form?
It's common. Urgent care visits and telehealth appointments often refuse employer paperwork, and a regular doctor's office may charge extra and only complete forms for established patients. That can mean waiting weeks for paperwork.

Is this a real evaluation, or paperwork from a template?
It's a real evaluation. The provider reviews your symptoms, dates, and limitations, and completes the form to what your situation supports. If it can't be medically supported, they tell you and you're refunded. Nothing is filled out from a template.
Can Certicare guarantee DLS will accept my form?
No. Amazon makes the final decision on leave and accommodation requests. Certicare can evaluate your case and complete the medical documentation supported by that evaluation. Submitting a form does not guarantee leave approval or a change to your UPT balance.
I already have a doctor's note. Isn't that enough?
Usually not. DLS forms ask for specific details: dates, functional limitations, a provider's signature, things a general note doesn't include. That gap is often why documentation gets marked insufficient.
Do I need a video appointment?

What if DLS asks for more information after I submit?

Yes, when the evaluation supports it. A state-licensed provider can complete paperwork today for a past absence, based on your intake and any records you provide. It's dated for the day it's completed, and it only covers what the evaluation supports.
