What if my doctor won't complete my leave forms?

You've probably heard one of these. The office doesn't complete employer paperwork. Forms are only done for established patients. The next opening is weeks out, and your form is due before that.

That refusal is the reason Certicare exists. A state-licensed provider reviews what you submit and completes the form to its requirements. There's no prior relationship required, and usually no appointment to book.

The review is a real review. A provider may follow up for more detail before deciding, and if what you describe isn't clinically supported, the form won't be signed. If we can't complete your request, for any reason, you're refunded in full.

What does my employer or leave administrator need?

For FMLA certification, employers may develop and request their own forms, and that's permitted as long as the form meets legal requirements. Using the form your employer sent is often the clearest way to provide every item requested and reduce follow-up.

"Insufficient documentation" usually means entries that are missing, vague, or nonresponsive — or a generic document, like a visit summary or discharge papers, sent in place of what was actually requested.

FMLA, a leave of absence, and an accommodation are not mutually exclusive. FMLA often runs at the same time as an employer's own leave process, and leave can itself be the accommodation. Send the form you were given; you don't have to sort out the tracks first.

Who can fill out my FMLA paperwork?

FMLA (Family and Medical Leave Act) recognizes a range of health care providers, not only physicians. Alongside doctors of medicine and osteopathy, the Department of Labor's list includes nurse practitioners, physician assistants, nurse-midwives, and clinical social workers acting within their scope of practice.

Certification can cover your own serious health condition, or care for a spouse, child, parent, or another qualifying family member. At Certicare, your documentation is completed and signed by a state-licensed provider.

FMLA has its own eligibility rules: roughly 12 months with your employer, 1,250 hours actually worked in the past 12 months, and a worksite with 50 or more employees within 75 miles. PTO and leave time generally don't count toward those hours. If you don't qualify, your employer's own leave process and accommodation requests don't require FMLA eligibility, and both use the same kind of provider-completed form.

Certicare completes the certification or leave form your employer or leave administrator sent you, field by field. If they write back wanting verification, clarification, or a correction, that follow-up is handled at no additional charge.

How fast can I get my leave paperwork completed?

Usually within 24 hours after we have everything needed: your questionnaire answers and the form you were asked to return.

For FMLA, employees generally have 15 calendar days after the employer's request to return a complete and sufficient certification. Go by the date on the notice you were sent.

The questionnaire is online right now. Email replies usually come the same day; anything sent after 6 pm ET is usually answered the next morning.
