Your likely starting point: FMLA

FMLA is a federal protection that can hold your job while you're out for a serious health condition — your own, or caring for a spouse, child, parent, or another qualifying family member. It generally applies after 12 months at Amazon and about 1,250 hours actually worked, a little over 24 hours a week; PTO and leave time usually don't count. Approved leave generally covers those absences instead of drawing down your UPT (Amazon's unpaid personal time). FMLA often runs at the same time as Amazon's own leave through DLS (Amazon's Disability and Leave Services), so an LOA, short-term disability, or an accommodation may apply too.

Your likely starting point: a medical LOA

A leave of absence through DLS is Amazon's own leave. It can cover time off for a medical reason when you don't meet FMLA's tenure or hours, and when it's approved, the absences it covers generally aren't charged to your UPT. FMLA and a medical LOA often run together, so if you've hit 12 months and about 1,250 hours, FMLA may apply too. DLS usually asks for a Health Care Provider Form, which a state-licensed provider can complete and sign.

Your likely starting point: an accommodation

An accommodation is a change that lets you keep working with a health condition — lighter duties, a schedule change, added breaks, or time off when that's the only option that works (Leave as an Accommodation). It fits ongoing or recurring conditions, and it can run alongside FMLA or a medical LOA rather than instead of them. Amazon reviews accommodation requests case by case. DLS uses a Leave as an Accommodation form, which a state-licensed provider can complete and sign.

Your likely starting point: short-term disability

Short-term disability replaces part of your pay while you're medically unable to work, often for a one-time event like surgery, childbirth, a serious illness, or an off-the-job injury. It's about income, so it usually runs alongside a leave rather than instead of one — many people file STD together with FMLA or an LOA. Whether you're covered depends on your plan and your state, so check your benefits in A to Z or ask HR what applies to you. If your claim asks for a Physician Statement, that's a form a state-licensed provider can complete and sign.

Your likely starting point: state paid leave

Some states run their own paid family or medical leave program, separate from Amazon. Where these exist, they can replace part of your pay while you're out, and they often run alongside FMLA or a leave through DLS. What's available, who qualifies, and how you apply depend entirely on your state's program. Your state's paid-leave or labor agency is the place to confirm the details and file.

Your likely starting point: workers' comp

If you were hurt or got sick because of your job, that usually goes through workers' compensation, not the leave paths above. Workers' comp can cover medical care and part of your lost pay for a work-related injury, and it's usually handled by an administrator such as Sedgwick, separately from a DLS leave. Report the injury to your manager promptly, since some states set short deadlines, and follow the instructions you're given on any forms and appointments. If you're also missing shifts, a DLS leave can run alongside it.
