This Looks Like FMLA
FMLA is a federal law, not a Walmart program. Once you've worked 12 months and about 1,250 hours in the past year, it can protect your job during qualifying leave, opened and tracked through Sedgwick.

This Looks Like a Medical Leave of Absence
PPTO covers a single, self-serve absence — no paperwork, no waiting. A Medical LOA is different: it's Walmart's own extended leave process, opened through Sedgwick, and it doesn't require FMLA eligibility.

This Looks Like an Accommodation
An accommodation changes how you work instead of stopping it — a schedule change, a temporary adjustment, or Intermittent Leave for a condition that flares unpredictably. Sedgwick still needs a provider's signature to process the request.

Can You Get Paid While You're Out?
Short-term disability can replace part of your paycheck after a 7-day unpaid wait: 50 to 60% of your average weekly wage, for up to 25 weeks if you're full-time hourly. Certicare's product doesn't file or complete STD claims.

This Looks Like State Paid Leave
Some states run their own paid family or medical leave program, completely separate from Walmart or Sedgwick. Whether one applies, and how much it pays, depends on the state you work in.

What If This Happened at Work?
This is a workers' comp claim, a different system with its own reporting steps and deadlines — but it can run alongside FMLA, so it doesn't rule out job-protected leave. Certicare's product doesn't cover workers' comp paperwork.
Report It to Your ManagerReport it as soon as possible — some states set short deadlines.
$49 covers this request: clinical review, the Sedgwick certification it needs, and any Sedgwick verification, clarification, or corrections on that certification — no additional charges. A later extension, return-to-work form, new condition, or other additional form is a separate request with its own fee. If your form hasn't been delivered yet, you can request a full refund.
