What does Sedgwick's Certification of Health Care Provider form actually need?

Its full name is the Certification of Health Care Provider for Associate's Serious Health Condition, based on the federal FMLA certification form. Four things make an answer complete:

  • Condition timing — the approximate date it started and its probable duration.
  • The kind of leave — continuous incapacity with an end date, a reduced or modified schedule, or intermittent episodes.
  • The medical basis — which condition category applies, plus any other relevant medical facts, like specialized equipment needs.
  • At least one essential job function the associate currently can't perform — not a checklist of every duty, just the one that's actually affected.

If the leave is intermittent, the form also asks for an estimated frequency and duration over the next six months.

Leave any of these blank or vague, and Sedgwick has grounds to send it back. None of this is instructions for what to write — it's what a clinician documents about a condition that's actually there. A limitation that isn't real can't be documented, by us or anyone else.

Why did Sedgwick say my FMLA certification was insufficient?

Usually it means the certification doesn't answer what the form actually asked. "Needs time off for medical reasons" describes a decision, not the specifics Sedgwick's form asks for: when the condition started and how long it's expected to last, what kind of leave it calls for, the medical basis, and at least one job function it affects. A generic visit summary or discharge note in place of that information reads the same way to Sedgwick: incomplete.

You don't legally have to use Sedgwick's exact form — federal rules say a certification can't be rejected just for being on the wrong form, as long as it has the required information. What vague vs. adequate looks like on this form

Vague (marked insufficient): "Patient needs time off work for medical reasons. Recommend leave until further notice."

Adequate: "Condition onset: [date]. Probable duration: [X weeks]. Continuous incapacity requiring treatment, [start date] to [end date]. Patient currently cannot perform [specific job function], an essential function of the role. If episodes continue after that window, estimated frequency: [X] times per month, [X] day(s) per episode, over the next six months."

What counts as an essential job function on the form?

The form asks for at least one essential job function the associate currently can't perform — not a full inventory of every duty, just the one (or more) that's actually affected by the condition. An essential function is a core part of the role, not an occasional task.

"Fatigue" or "pain" on their own aren't functions Sedgwick's form can use — they're symptoms, not job functions. Tied to an actual duty, like can't stand for a full shift or can't lift a stated amount, a clinician can document it as a specific, current limitation on a specific function.

How do I document intermittent leave frequency for Sedgwick?

For a condition that flares rather than sidelining you all at once, the form asks for an estimated frequency and duration over the next six months: how many episodes, and how many hours or days each one is expected to last.

An estimate is enough. Sedgwick isn't expecting a crystal ball. It does need a number attached to that six-month window, not "as needed" on its own.

Does my doctor need to name my exact job duties?

Not your job title, and not every duty — just the one essential function the condition currently affects, in plain terms a clinician can tie to a specific limitation. A clinician who's never set foot in a Walmart facility can still complete this accurately, because you're the one who knows which part of the job the condition affects; turning that into documentation is the clinician's job.
