If you were hurt on the job at Amazon, this isn't your page. On-the-job injuries run through workers' compensation, administered by Sedgwick — a different process with different paperwork, and Certicare doesn't handle it. Workers' comp cases are screened out at our intake, so we'd rather tell you now than after you've started. Does Amazon actually have light duty?
Not as a guarantee, no. Whether your building can offer modified tasks, and which tasks, is Amazon's internal call, and it changes from site to site. Amazon makes the final decision on leave and accommodation requests.
The route runs through DLS. You open an accommodation case, in A to Z or by phone, and DLS decides it based on paperwork a licensed provider completed about your restrictions. Sometimes the answer is modified tasks; sometimes it's something else, like leave used as the accommodation.
Your side of it is the paperwork. That's the part Certicare does.
Can Amazon fire me for lifting or standing restrictions?
The honest answer: it depends, and no form makes a job untouchable. The ADA requires employers to consider reasonable accommodation for a qualifying disability, and pregnancy-related restrictions fall under the Pregnant Workers Fairness Act, in effect since 2023. Whether those protections cover your situation is a legal question, and this page is not legal advice.
In practice, the risk workers describe isn't a firing over the restriction itself. It's attendance: UPT dropping while a case sits open, because DLS decides the case but HR and timekeeping own UPT and points.
So keep a record. Screenshot your A to Z balance and save every DLS message with a date in it. Then get your paperwork in by the deadline in your DLS message — a case that's waiting on documents is the case that closes.
What form does my doctor need to fill out for restrictions?
Usually one of Amazon's own: the Health Care Provider Form, Leave as an Accommodation paperwork, or a Physician Statement. An RFI is a request for additional information and can follow any of them. A plain note with your lifting limit on it is what workers report coming back as insufficient. Using the DLS form is usually the clearest way to provide every item requested and reduce follow-up.
Certicare's part is that form. You answer a short online questionnaire, usually with no appointment to book, and a state-licensed provider reviews your answers, then completes and signs the exact form your case calls for. A provider may follow up for more detail before deciding.

What happens after I submit my restrictions?
DLS reviews the case and decides. You can watch it in A to Z: MyHR → My Requests → Open cases → your leave case. Go by the deadline in your DLS message, not by what a forum thread remembers.
DLS sometimes writes back with a question or an RFI. Certicare handles that follow-up at no additional charge.
If the case is approved, the accommodation should flow through to your schedule and attendance. It may not happen automatically. Check your UPT after the decision, and if the numbers don't match the approval, raise it with HR.
