What if my employer does not have a form?

We can usually provide a signed letter that explains the recommended work accommodation.

Do I need a video visit or in-person appointment?

No. Most requests are handled through our secure online intake and email.

How fast will I get my paperwork?

Most completed forms or letters are delivered within 24 hours.

Can you send the paperwork directly to HR?

Yes. If you want, we can email the completed PDF directly to your employer or HR contact and copy you.

What if my employer asks for changes?

We provide updates to the form or letter for 14 days after delivery at no extra cost.

Will every request be approved?

No. Requests are reviewed by a licensed provider. If the requested accommodation is not medically appropriate, we may decline the request. In this case, you will receive a full refund.

What information is included in the paperwork?

Your paperwork may include the provider’s name, credentials, license information, signature, date of evaluation, and the recommended accommodation. We only include the information needed for your request.
