What is the Family and Medical Leave Act (FMLA)?

FMLA is a federal law that lets eligible employees take up to 12 weeks of unpaid, job-protected leave each year for their own serious health condition, to care for certain family members, or for a new baby or adoption. Your job (or an equivalent one) must be waiting for you when you return.

What is New York Paid Family Leave (NY PFL)?

NY PFL is a state program that gives most private-sector workers up to 12 weeks of paid leave (about 67 % of your average weekly wage, up to a state cap) to bond with a new child, care for a seriously ill family member, or handle certain military-related duties.

What is New York Paid Sick Leave?

Under the NY Paid Sick Leave law, employees earn 1 hour of paid sick leave for every 30 hours worked.

Employers with 1–4 workers and ≤ $1 million in annual revenue must provide 40 hours unpaid (but paid if revenue exceeds $1 million).

Employers with 5–99 workers must provide 40 hours paid.Employers with 100 + workers must provide 56 hours paid.

You can use this time for your own illness, preventive care, or certain family-care needs.

Am I eligible, and which forms do I need?

The safest first step is to ask your HR department:

“Am I eligible for FMLA, NY PFL, Paid Sick Leave, or other benefits?”

“Which specific forms must my healthcare provider complete?”

Once you know the exact paperwork, start our intake and we’ll handle the rest.

What health conditions count as “serious” for FMLA?

Common examples include:

Surgery recovery (e.g., knee replacement, appendectomy)

Pregnancy and prenatal/post-partum care

Autoimmune disorders such as lupus, rheumatoid arthritis, multiple sclerosis

Gastro-intestinal diseases like Crohn’s disease or ulcerative colitis

Diabetes or its complications

Migraine or chronic severe headaches

Mental-health conditions
requiring treatment (major depression, anxiety)

Asthma or COPD flare-ups that need medical intervention

If you’re unsure, start the intake—our provider will confirm whether your situation meets the FMLA definition of a “serious health condition."

How fast will I get my completed forms?

Within 24 hours.

Do I need a video visit or in-person exam?

No. Most cases are handled through our secure online intake; we’ll phone or email you only if we need extra details.

Can you send the paperwork directly to my HR or insurance carrier?

Yes. On request we email the signed PDF to them (and cc you) at no extra cost.

What if HR asks for changes?

You get unlimited revisions for 14 days after delivery. If the paperwork is still rejected, we’ll refund you in full.
