Can you get a doctor's note for mental health?
Yes. A mental-health absence is an absence. If anxiety, stress or low mood kept you from working, the note that covers those days works the same way as a note for a cold or a stomach bug: a state-licensed provider evaluates you and decides whether the evaluation supports the dates. Nothing about the reason for the absence changes that process.
What the evaluation looks at is narrow. It asks whether you were unfit for work on the days in question, and whether those dates fit the note's limits. It is not a ranking of how unwell you were, and it is not a judgment of you. The provider is deciding whether the evaluation supports the dates.
A provider can decline. Where the evaluation does not support a note for anxiety, stress or low mood, the provider does not write one, the same as for any other condition. Payment does not guarantee a note. If the note is not written at Certicare, you are refunded in full.

What does a mental-health doctor's note say?
Less than you may expect. A note says that you were evaluated by a state-licensed provider, the dates the absence covers, whether you are fit to return and when, and any restrictions. It does not say what the diagnosis was, what your symptoms were, or what treatment you had, unless you ask for that to be included.
The diagnosis is the part people worry about, so here is the rule in full. Your employer does not see the diagnosis, unless you have specifically requested that it be on the note. The note does not carry the diagnosis on its own, and if you decide you want it there, you can ask for it and it goes on. For a mental-health absence, that means the note reads like a note for any other short illness, and the reason behind the dates is yours to share or not.
The note itself is yours. Kept private and never sent to your employer or school without your say-so. If something on it needs correcting: where we wrote the note, the correction is included and you do not pay again.

Is a mental health day a sick day?
A doctor's note certifies an evaluation, not a day off. That distinction answers the question. If you were not fit for work because of anxiety, stress or low mood, that is an illness absence, and a note can cover it where the evaluation supports it. A rest day taken while you were well is not something a note certifies, because there is no illness for the evaluation to find.
If what you searched for was a doctor's note for a mental health day, the evaluation decides which of those two things your day was. The intake asks what was going on and which scheduled days it cost you, and the provider decides whether the dates are supported. Where the evaluation supports the dates, the note is written, dated the day it is signed. Where it does not, it is not written and you are refunded in full.
Your workplace may or may not count that day as a sick day; that is a separate question from whether a note can cover it. Sick-leave policies and sick-leave laws can cover mental as well as physical illness. Your own policy, or the law where you work, sets that, and a note does not change what your policy says. What the note does is document the absence, so that if your policy treats it as sick time, the paperwork is there.

When is it a leave question instead?
A note covers a short absence. More than a few days, a pattern that keeps coming back, or a condition that keeps you from working for weeks is a leave question.
The line between the two is the shape of the absence, not the condition behind it. A day or two off for anxiety is a note. The same anxiety keeping you home for weeks, or coming back again and again, is a leave question. The note's limits, up to 3 scheduled workdays and 6 in any 30-day window, are a practical marker: an absence that will not fit inside them is one the note cannot document, and leave paperwork is the next thing to look at.
