No. Every note is written and signed after a licensed clinician reviews the symptoms you submit.  No blank templates or auto-generated PDFs are ever issued. These are official medical documents signed digitally by licensed healthcare providers.

Will my employer/professor accept this note?

Employer verification support is included. If your employer has questions, they may contact us using the information listed on your note. Acceptance depends on your employer’s policy.

How do employers or schools verify the note?

Your employer or school can contact your provider directly, using the phone number or email printed on your note.

Do you ever decline requests?

Yes.  Roughly 1 % of intakes are denied because the reported symptoms do not justify time off or require in-person care.

Do I need an in-person exam or video call?

No. This is an asynchronous service, meaning all communication happens via secure forms and email.

If you have severe symptoms, an in-person visit may be recommended instead. Your employer or school can contact your provider directly, using the phone number or email printed on your note.

What kinds of health conditions can I get an doctor's note for?

Can I request a note for my child?

Yes, parents or legal guardians over the age of 18 can request a short-term note for their child.

Simply enter the child’s name, date of birth, and relevant health details when completing the intake form.

Each note contains:
