No. This is an asynchronous service, meaning all communication happens via secure forms and email.

If you have severe symptoms, an in-person visit may be recommended instead.Your employer or school can contact your provider directly, using the phone number or email printed on your note.

What kinds of health conditions can I get an doctor's note for?

Our service is designed for patients with mild, self-limiting conditions such as common respiratory viruses, stomach upset, and symptoms of previously diagnosed chronic conditions like migraines.

Can I request a note for my child?

Yes, parents or legal guardians over the age of 18 can request a short-term note for their child.

Simply enter the child’s name, date of birth, and relevant health details when completing the intake form.

What’s included in the doctor’s note?

Each note contains:

•      Provider’s full name, credentials, NPI number, and signature
•      Provider’s direct phone number and email
•      Evaluation date
•      A brief reason for the absence (e.g., “illness”)
•      Up to 3 recommended excused days
•      Anticipated date of return to work or school

We do not share sensitive health details or in-depth medical history, to protect your privacy.

Is this a subscription?

No, our service is not a subscription. You are only charged once for each note you request.
