Requesting an accommodation at Amazon — the full walkthrough
Each step of Amazon's accommodation process, in plain terms — and when DLS asks for provider paperwork, a state-licensed provider completes and signs it after a real evaluation.

You open it yourself, in A to Z. Start a new request and describe, in plain words, what's making the job hard — an injury or a condition, and what it stops you from doing. You don't need medical language to open the case.
The request routes to DLS, and a DLS accommodation consultant is assigned to your case. To find the case again later in A to Z, the safest path is MyHR → My Requests → Open cases. Your case number is on formal DLS emails, letters, and provider forms, and inside the request itself in A to Z; it may not show on every notification.
From day one, save anything with a date or a deadline in it. Screenshots count.
What will the DLS accommodation consultant ask me?
Expect questions about your job as much as your condition: which tasks are hard for you, and what change would let you keep working. If a provider has already given you restrictions, have them handy. You're describing your shifts, not writing a medical chart.
At some point the consultant will usually ask for documentation on a specific form. For accommodation cases that's often a Physician Statement or Leave as an Accommodation paperwork, and an RFI is a request for additional information that can follow any form. What the case is usually waiting for is the specific form, not a generic visit summary.
Answer what you know. The medical part belongs to the provider, and that part comes later on this page.
What is the "interactive process"?
"Interactive process" is a legal term, not an Amazon one. It comes from the EEOC's ADA regulations, and EEOC guidance describes it as the back-and-forth between you, your employer, and your provider's documentation. In practice it means the case is a conversation. You describe the limitation, DLS asks questions, and the provider paperwork answers the medical part.
Interactive also means more than one round is normal. An RFI means DLS wants more information before it decides. Getting one doesn't mean you've been denied.
The tracks also overlap: FMLA, LOA, and accommodation can run together, so opening one doesn't lock you out of the others. If your case belongs on a different track, DLS can move it; your part stays the same.

What accommodations does Amazon actually grant?
DLS decisions describe a few kinds of outcomes: modified tasks, equipment, schedule changes, reassignment to another role, or leave used as the accommodation itself — that last one is what "Leave as an Accommodation" paperwork exists for. Which of these you're offered, or whether any is, is Amazon's call. No form, and no service, changes that.
One thing catches workers after an approval: DLS decides the accommodation, but HR and timekeeping own UPT and schedules. An approved accommodation should flow through to your attendance record — it may not happen automatically, or immediately. If your balance doesn't match the decision, raise it with your case number and dated messages in hand.

How long does an Amazon accommodation decision take?
There's no public Amazon document that states a DLS accommodation timeline. Workers report waits that vary widely: some hear back in days, others wait much longer. Anyone quoting you an exact number is guessing.
The part you control is your own deadline. Your DLS message names the date your paperwork is due — go by that, not by what a forum thread remembers. A case that's waiting on documents is the one at risk of closing.
While you wait, the case itself is the source of truth. Check it in A to Z rather than counting on notifications, and keep dated screenshots of anything that shows your balance or a deadline.
Do I need my doctor to fill out the accommodation paperwork?
Someone licensed does, yes — and it doesn't have to be your own provider. The forms DLS sends (Physician Statement, Health Care Provider Form, Leave as an Accommodation paperwork) are usually what the case is waiting on, and each is completed and signed by a state-licensed provider, field by field. "My doctor won't fill out the form" is one of the most common lines in worker threads; urgent care and telehealth apps often refuse employer paperwork outright.
